Camper Information Form

This is not a registration form, and must be completed after you have registered online.

Please note: Only one form per child is required, regardless of how many weeks your child is registered for.

Field Trip Permission Form

This is a general permission form for all field trips scheduled throughout the Camp JCC summer program.

Forms are fillable and need to be completed and emailed to our camp office.
